Girls and guys, what are the best and worst ways to ask a girl out?

Well I'm a girl, so I had a boyfriend who asked me out over text. I mean, it was okay with me; I wish he wouldve asked me in person, but he was kinda shy. Just know that doing that can turn a girl off or even piss her off. I think a guy should ask to hang out, maybe at the mall or at the movies and then near the end ask her. That'd be cute :)
